RUBRIC REQUEST FORM

PACS:   Are you considering using rubric evaluation as a Program Learning Outcome (PLO) assessment tool this semester? To determine how well our PLOs are being achieved, programs may choose to include evaluation of student artifacts such as papers, portfolios, or performances. A rubric can then be a very useful tool for assessing student learning as reflected in these products. Using a rubric for program assessment is different than using a rubric for grading student work. For the purpose of assessment, scores for each dimension of the rubric are aggregated across all students. The objective is to understand the average skill level of the students in the program on one particular dimension.

Complete the form below to have your rubric attached to the roster of a course you would like to use for assessment.  We will then load your rubric and course roster to the Outcome Assessment Projects software which will all your instructors in your to access the rubric to score an assignment for assessment purposes.  We will then be able to provide you with reports from that data using demographics and share the reports into the Assessment Results section of Planning and Self-Study.
