You should have received them via email, on your personal email, after you made your deposit. They are also located on our website: bw.edu/admitted. Once on the website, scroll down to the fifth yellow category labeled "Onboarding Forms (Housing, Class Schedule, and Orientation" and log in to access your forms.
Form Descriptions:
Orientation Scheduling Form
This is the form you will be using to choose which orientation session you will be attending. We recommend you fill this out ASAP since options for sessions are removed as the sessions fill up.
Schedule Preference Form
This form contains a personal inventory of your class preferences as well as which classes you would like to take. You will also have the option to submit ACT/SAT scores and opt-in to take placement tests. This will allow BW Academic Advisors to prepare a fall semester schedule for you.
Health History Forms
This form is NOT mandatory, however, we highly encourage you to fill it out. It gives the Health Center insight on your medical and immunization history to best help you!
Resident/Commuter Housing Form
This form is simply a form to declare if you will be living in the residence halls or choosing the commuter life your first year. If you are going to live in the residence halls, the sooner you fill this out will determine your placing to select housing in the summer.
Mobile Technology Agreement
This form describes the various technology policies that BW and it requires a signature agreeing to abide by these policies! It is super quick and easy to fill out.

As a deposited student, these forms are your next step into becoming a student and joining the BW community!
