BW Student Athletes Orientation

The list of outstanding medical requirements is below:

Medical Paperwork through Healthy Roster

• You will be sent a link in a separate email to set-up an account
• This will go Live on June 1, 2026
• This should have come from Healthy Roster directly (search Healthy Roster in your BW email and spam folder if you cannot find it)

Forms that Need Completed:

• Liability Waiver and Medical Release
• Secondary Athletic Insurance Acknowledgment
• ADHD Medical Exceptions Notification
• Authorization to Obtain and Release Medical Information
• Combined Medical History: Incoming Athlete
• Concussion Awareness
• Consent to Treat
• Primary Insurance Information
• Patient Health Questionnaire (PHQ)
• Sickle Cell Information & Documentation
• Substance Abuse/Misuse and Consent to Drug Testing

Forms that DO NOT Need Completed:

Physician Form for PPE
       • This will be completed during your physical with our team doctors here on campus

SWAY Concussion Testing

• Testing is done on your phone, on your own, through the SWAY app
• Instructions can be found below
• This is our baseline concussion testing
• Should take 20-30mins to complete and needs to be done in one sitting, so please plan accordingly


Sickle Cell Testing

Completed on your own prior to coming to BW - new born records will not fill this requirement

• Can be done at a physician's office and some urgent cares and Quest Labs
• Results should read as Negative or Positive and attached to your Sickle Cell Information and Documentation Form

New Athlete Physical Sign-Up

Use the link provided below to sign-up for your new-athlete physical

• The slots are for every half an hour, but please prepare to be there for at least an hour
• Please bring your laptop or tablet to physicals as you will need to use it during several stations
• Please make sure that you are signing up for the physical date that correlates to your team or competition season
• Men's Soccer, Women's Soccer, Men's and Women's Cross Country, and Women's Volleyball do not need to sign-up for a physical time as they will complete their physicals with the rest of the incoming athletes on their team on the day that they report to campus for pre-season. Times will be given to athletes of those sports at a later date by their Head Coaches

During the REQUIRED Student Athlete Session during Orientation you will take care of the following:

• Finish any and ALL outstanding medical paperwork
• Be briefed on the secondary insurance policy that applies to all student athletes that sustain an injury at Baldwin Wallace
• Be briefed on the medical resources that will be available to you during your time at BW
• Ability to ask any questions or express any concerns regarding your sport or your medical paperwork
